Bright Heart Health offers mental health outpatient care to individuals with mood, personality, and other mental health issues. Patients meet with providers in both individual and group sessions. Each patient is assigned a multi-disciplinary team of professionals who take a patient-centered approach to care.

Outpatient Care

Our outpatient model is designed to provide comprehensive support to individuals using evidence-based care. Our care model incorporates a metrics-based approach utilizing well-validated psychomeasures assessing all aspects of a patients care progress. Depending on the patient's needs, physicians, therapists, mental health counselors, dietitians, case managers, and wellness coaches work with patients on their treatment goals. Patient's presenting with co-morbidities may work specialized providers to address trauma, pain, substance use disorder, or other issues.

Post Hospitalization

Bright Heart Health offers a post hospitalization program designed to provide convenient and easily accessible care within the 7 and 21 day follow up period. This program is specifically designed as a low-threshold care model so patients can been seen quickly to support their needs. Bright Heart is actively working with hospitals to provide ongoing care to their patients who are discharging and need follow up care.

    All sessions are conducted via video conferencing. Individuals can join by computer, tablet, or smart phone.

    Individuals join from the privacy of their own home. Patient have the flexibility to meet with providers during the day or evening.
